We report the case of a 61-year-old man, with Waldenstrom’s macroglobulinemia, who was hospitalized for paresthesia and weakness in lower and upper extremities; during hospitalization he developed acute ischemic stroke for which he received fibrinolytic therapy with rtPA with complete remission of his symptoms; six hours later he developed a new ischemic stroke with progressive aggravation and death.
