Cardiovascular diseases (CVDs) account for 46% of deaths in Iran, predominantly affecting the elderly. These diseases impose a substantial financial burden on Iran’s healthcare system. Predicting the risk of cardiovascular events based on known risk factors is vital for implementing preventive measures. Accordingly, the present study aimed to assess CVD risk using the World Health Organization (WHO) chart and its associated factors among older adults covered by comprehensive health centers in Rasht in 2022. This cross-sectional study was conducted on 379 older adults who attended Rasht health centers, selected through stratified random sampling. Data were collected using demographic questionnaires and lifestyle-related risk factors, as well as anthropometric and metabolic indices. The WHO/International Society of Hypertension (ISH) risk prediction chart was applied to evaluate CVD risk. Among the participants, 57.3% had a low risk, 31.9% a moderate risk, and 10.8% a high to very high risk of CVD. Surprisingly, diabetic individuals showed a lower risk compared with non-diabetic participants (83% vs. 46.4% in the low-risk group, P<0.001). Significant risk factors included age, sex
