Over a long career in both ablative and reconstructive surgery in the head and neck, the author has identified certain factors which help in the management of these patients and has learned many lessons.Such lessons include the importance of multi‐disciplinary management, excellence in anaesthesia, the importance of anatomy, concepts of ablative surgery such as marginal mandibular resection, and many of the concepts of reconstructive surgery which have enabled rapid one stage reconstructions, such that palliative surgery for massive tumours can be sometimes worthwhile.These and other experiences will be discussed.
