Abstract Aims The application of robotic-assisted surgery for colorectal cancer in octogenarians remains under-explored compared to laparoscopic surgery. This study aims to compare short- and long-term outcomes of robotic surgery to laparoscopic outcomes in Octogenarians published in the study by Rossi et al. Methods A retrospective review of all octogenarians, who underwent elective colorectal robotic resection, between August 2021 and October 2024 in a single hospital. Data was compared with laparoscopic resections. Chi-Square test was used to compare categorical variable. Results A total of 25 patients, 52% (13/25) males, with a median age of 81 (range 80-88) years, were operated in the robotic group. Median length of stay (LOS) was 8 days. 74% patient underwent rectal surgery comparing to 35% in laparoscopic group p=0.001. Robotic colorectal resection had a lower rate of postoperative complications (32% vs. 44.5%) p=0.18 reduced 90-day mortality rate (4% vs. 5.5%) and rate of R1 resection (4%
