Abstract Inferior vena cava (IVC) filters are commonly used to prevent pulmonary embolism from deep vein thrombosis in select patients. However, certain filter morphologies can increase case complexity and compromise the safety and feasibility of retrieval. We present three unique cases of a Mobin-Uddin filter, bird's nest filter, and an embedded filter where retrieval was deferred and the patient was successfully treated with endovascular stent reconstruction. Reconstruction across the IVC filter and within the iliac veins restored venous patency with resolution of symptoms and sustained improvement at 1-month follow-up. These cases demonstrate the feasibility of endovascular stent reconstruction for complex IVC filter-associated venous obstruction.
